Responsibilities for thePrimary RNinclude:
Assessing and admitting patients to the services of Hospice of NGMC
Collaborating with attending physician to develop and implement the appropriate plan of care
Providing and supervising the care that is given to patients and their families, ensuring that it is delivered in a manner that preserves and protects the patient's autonomy, dignity and rights
Coordinating all aspects of care for patients and their families through communication with appropriate members of our interdisciplinary team as well as various community agencies/resources as indicated

Care is provided with respectful compassion to our patients in their home and members of the care team includes the patient's physician, RNs, social workers, bereavement counselors, personal care aides as well as others.
