Job Details: Associate's Degree RN - Registered Nurse 1 Year of Experience Required Working collaboratively with the physician and other members of healthcare team, supports patient care monitoring, coordination, and facilitation of the patient care delivery system integrating clinical and financial coordination and data management.Promotes quality outcomes, team accountability, flexible, productivity and becomes a link between the patient, the provider, the payor, and the community.Responsible for discharge planning, continuum placement, access control, and advocate for the patient in receiving quality, cost-effective services while protecting the interest of the organization.This position will come in contact with patients in the neonate, infant, child, adolescent, adult, and geriatric age groups.Employee performs clinical duties in accordance with population specific guidelines and adheres to National Patient Safety Goals as outlined in policy and procedures. Applicants to this position must have a minimum of one year recent experience as a RN in the field of hospital based Case Management as well as a minimum of three years experience as a RN in Adult Medical-Surgical, Emergency Department or Critical Care environments. The schedule for this position will be vary weekend coverage.
